The pharmacy department is responsible for the control and proper use of medications in the emergency
room, ambulatory care unit, surgery unit, inpatient units, and SPMH owned clinics. A pharmacist will
monitor all physician orders for accuracy and safety. This will include screening for correct dosing
based on age or status of illness, potential drug interactions, possible allergic reactions,
therapeutic duplication and appropriate directions for administration. Pharmacists respond to
medication questions utilizing up-to-date drug information resources. A pharmacist is available for
consultation 24 hours a day.
Pharmacy Anticoagulation Management Service
The anticoagulation management service (AMS) is a specialized program focused predominantly on
managing oral anticoagulation (Coumadin®). The program is the responsibility of the Pharmacy
Department and functions under the guidance of the AMS supervising physician. Clinical pharmacists
with specific knowledge in anticoagulation therapy provide this service by meeting one-on-one with
the patients to educate them about their medications, monitor therapy, adjust dosing based on lab
values if needed, and report any changes or problems with the patient's physician. This coordinated
anticoagulation management service appears to offer important qualitative and cost advantages over
routine medical care by improving therapeutic effectiveness and reducing complications,
hospitalizations, and emergency room visits. The AMS will monitor any patient upon written request
for consultation from a SPMH hospital or clinic physician.
